Waimakariri Practice Enrolment Waitlist

We are currently accepting WAITLIST REQUESTS for people who live or work in the Waimakariri region.

IMPORTANT: THIS IS NOT AN ENROLMENT FORM.
Submitting this form does not enrol you with our practice. It only places you on our enrolment waitlist.

Eligibility

• You may join the waitlist if you live or work in the Waimakariri region.
• You must not currently be enrolled with another general practice in Waimakariri.
• The current estimated waiting time is approximately 4–6 weeks.

What happens after you join the waitlist?

When a place becomes available for enrolment at either our Woodend or Rangiora practice, we will contact you.

At that time, we will send you an electronic link to complete the actual enrolment form.

Please do not consider this form an enrolment application. It is a waitlist form only.
